An Endoscopic Endonasal Approach to Craniopharyngiomas: Surgical outcomes and follow-up results of 150 patients

Background: Craniopharyngiomas are locally invasive neoplasms and usually f irmly adhere to the surrounding neurovascular structures. Despite advancements in endoscopic techniques, gross-total resection (GTR) of tumors with invasion or adhesion to important surrounding anatomical structures is extremely difficult. We present a single-center study that evaluated the impact of the endoscopic endonasal approach (EEA) on the surgical outcomes of craniopharyngiomas, the factors affecting the resection rate, and recurrence. Methods: 170 operations on 150 patients with pathologically identified craniopharyngioma who were treated via the EEA from August 1997 to January 2024 were included in the study with the added cases after the articles that were published in 2021 and 2023 (1,2). Results: In total, 45 patients (20 pediatric and 25 adult patients) (30%) had history of operation before. The most common symptom at presentation was endocrine disturbances (94.6%). Patients were classified as infrasellar (6%), sellar (33.3%), sellar-suprasellar 64 (42.6%), and suprasellar (18%) localization. GTR was achieved in 103/150 patients (68.6%). The ratio of GTR was higher in infrasellar (100%) and sellar (90%) tumors compared with sellar-suprasellar (62.5%) and suprasellar (33.3%) tumors. Postoperative complications, other than endocrine impairment, occurred in 24/150 patients (16%). At follow-up, 45/150 (30%) patients presented with tumor recurrence (27 pediatric and 18 adult patients). Conclusions: This retrospective cohort study provides surgical indications for EEA, as well as the factors affecting the resection rate and recurrence of craniopharyngioma. The most important factors affecting a successful resection are recurrent tumor, the location of the tumor and increasing experience.
