Photopenic lesions, particularly small ones, may prompt false-negative interpretations of bone scans and sometimes can be overlooked, decreasing the sensitivity of the bone scan in the detection of bone metastases. The authors report photopenic bone metastases of carcinoma of the larynx to the left femur and right iliac wing detected by dual imaging using Tc-99m MDP and Tc-99m MIBI.
