The respondents, amid concern that responses may be known to others in cases such as data security, anonymity, traceability, and transparency, usually object to honest answers. Because of that, the results of surveys can be wrong, and the people, companies, etc. who prepare the survey make the wrong decisions. The problem of changing survey records for various purposes not only creates inaccurate outcomes but also can result in big problems. In this study, the anonymity feature of Blockchain technology and its benefits and negative sides in surveys are assessed. First of all, a draft structure was formed so that services were defined and data and process flows were planned. The draft structure has also been recommended as a distributed system on the open-source Hyperledger Fabric network. A detailed process that is fulfilled by establishing confirmation bias is recorded in all distributed ledgers; all relevant survey responses are thus traceable, questionable, and verifiable.
